We’ve streamlined our patient form process with Mobile Pre-Check! This will allow us to spend more time on you, not your paperwork. As your appointment approaches, you will receive an email or text notification from Wellish Vision Institute, which is an affiliate of American Vision Partners.

This notification is to confirm your appointment and complete your registration paperwork prior to your visit. Please ensure that you provide an accurate phone number and email address when you schedule your appointment to ensure that we are able to get in contact with you.

It’s simple. Here’s how it works:

  1. Click the link in your email or text message to confirm your appointment.

2. Complete the Mobile Pre-Check process via your home computer, tablet, or smartphone and your check-in will be expedited on the day of your appointment.
